The Everharts
After 2 years of infertility testing and treatments, Jordan and I began to think that God had something planned for our lives other than biological children. We chose open adoption to grow our family and we couldn't have imagined how it would transform our lives and those around us in such a wonderful way. This is our journey to a different kind of family...one that we wouldn't trade for anything!

The Complete Process
Pre Application
-questionaire
-1/2 day orientation at agency
Application-$300
-14 page questionaire
-drivers license and criminal history check
-adoption fee agreement
-firearms safety verification
-copy of birth certificates
-copy of social security cards
-copy of driver's license
-copy of marraige license
-verification of Employment
-copy of Tax Returns for last 3 years
-affidavit of Health Insurance Coverage
-Copy of Life Insurance (min $50k each spouse)
-Emergency phone numbers
-Map to Home from adoption agency
-Floor plan of home including square feet of each room
-Copy of pet vaccinations
-Physicians report(physicals)
-4 reference letters to be mailed directly to agency(3 non-family who have known couple at least 3 years, 1 from church leader)
-Ten year residency history
-Current credit report for each spouse
-Statement of Understanding: No guarantee of placement
-Agreement to Terminate services
-Waiver of Confidentiality
-Acknowledgement of Notice to Require testing
-Policy of Ethical Practice -Picture of Husband and Wife
-Two pictures of home (front and back)
-HIPPA releases
Office Interviews-$500
-assigned social worker meets individually and together
-begin required reading (3 adoption books)
2 Day Seminar-$300
-discussions and group exercise -read additional seminar workbook
Home Study-$500
-social worker conducts safety visit to home
-additional interviews at home
-complete draft of Picture Profile/Relationship
Profile -verification forms
-evacuation plan/safety checklist
Entrance into Book-$2,000
-finished Picture Profile for birth parents
-website Profile completed and published
Phone call -Birth parent will contact via phone
Match Meeting
-meeting with birth family at Agency
-24 hours for both parties to consider match
Ongoing Contact after Match -
get to know birth family outside of agency
-scheduled agency meeting
Hospital/Birth
-Agency facilitated plan to visit hospital as birth family suggests
Relinquishment/Entrustment-$21,400
-sign documents
-Entrustment Ceremony for adoptive and birth families at hospital
Finalization!
-6 months after placement court date with Lawyer

Did we make the friend cut?!!! Yay! :) I can't see it to clear but I can make out "Trip to CA" and my hubby's shirt. Awww, love you too lady!
